Add links to navigate back to the table-of-content.

But which one? Every chapter has a chapter-toc, and there is the global-toc.

Bug #28606 asks to split the huge HTML file into multiple chunks.
Bug #29401 complains about the HTML toc being too long.

Documents were transformed to reStructuredText. The table of contents is now always visible on the left and allows easy documentation within the document.
