A diagnose module could check that a mail servers external up IP address will be resolved to an FQDN that in turn is resolved to the same IP address.

After a long discussion with Daniel, we came to the conclusion that such a diagnostic module will most likely not cover all possible scenarios.

We can not know that the hosted domain is also the mail server which both receives and sends mails.
